Delivery Information:
Orders of one pack of 50 forms will usually be shipped Priority Mail, and all other orders go
UPS ground.
Within the Continental US, freight charges are as follows:
| Orders less than $75 will be billed $5.00 in freight. |
| Orders between $75 and $100 will be billed $9.00 in freight. |
| Orders $101 - $200 are billed $12.00 in
freight. |
| Stock orders over $200 are billed a flat $19.00, regardless of your location within the continental US. |

Expected delivery times are based on In-Stock items. If an item is Out-of-Stock, we will call or email you within 24 hours of your placing the order.
Expected Delivery Times:
